D4: Practice Unit Information Forms

There are four Practice Unit Information forms

Initial Notification – PUI1 – this is the appropriate form if you require to notify details of a new practice unit.  The form should be returned to the Society no later than seven days after the new practice unit commences trading.

Annual Confirmation – PUI2 – this is the form which we issue to practice units pre-populated with the information held by the Society. When you receive one of these forms you should check and correct the information as necessary and return the signed form within 28 days of its receipt. You should not need to download and complete a blank form - it is included here purely for information.

Notification of Change – PUI3 – this is the form you should use to notify us of a change to the relevant information held by the Society.  For example, changes to the individuals designated as Cashroom Manager (Designated Cashroom Partner), Client Relations Manager (Client Relations Partner) etc can be intimated using a PUI3 form. 

Notice of Cessation of Practise – PUI4 – this is the form which should be used to notify us of the intention to cease trading. It should be completed and returned to the Society no later than 28 days prior to the planned cessation of trading.
 

Fees – there are no fees applicable to any of the Practice Unit Information forms.
